1st Cavalry Division arrived in South Vietnam and the 5th Cavalry was soon in action; from 18-20 September, 2-5 Cavalry supported friendly elements in Operation Gibraltar. However, their first real test did not come until the Battle of Ia Drang.[10] During the fight for LZ X-Ray, elements of the 7th Cavalry were surrounded by large People's Army of Vietnam (PAVN) forces, and 2-5 Cavalry arrived to help. They arrived on the American line and quickly engaged enemy forces, eventually rescuing the "lost platoon" led by SGT Ernie Savage. Once the battle was complete, the 2/5th Cavalry troopers set out for LZ Columbus while Alpha Company 1/5th Cavalry and 2/7th Cavalry set out for LZ Albany. During the Vietnam War, the 1st Battalion, 46th Infantry and the 5th Battalion, 46th Infantry were part of the Americal Division. Both initially deployed to Vietnam with the 198th Infantry Brigade from Fort Hood, Texas. But the 1-46th was placed under operational control of the 196th Infantry Brigade (Light) and later became fully a part of the 196th Brigade. Though the 5-46th left Vietnam when the Americal Division cased its colors in the fall of 1971, the 1-46th deployed to Da Nang with the rest of the 196th. The 1-46th left Vietnam in June 1972.

In 1965, the 1st and 2nd Battalions were deployed to South Vietnam with the 1st Infantry Division. 2nd Battalion were badly mauled during the Battle of Ong Thanh in October 1967. Early 1970 saw the end of the participation of the 1st and 2nd Battalions of the 28th Infantry in this war.

Three battalions, the 1st, 2nd and 5th, served during the Vietnam War as the 3rd Brigade of the 1st Cavalry Division. 3rd Brigade often referred to itself as the "Garryowen Brigade". These troopers were armed with the new M16 rifle, M1911A1 Pistols and the M79 grenade launcher. The use of Bell UH-1 Iroquois "Huey" helicopters transformed the 1st Cavalry into an "Air-mobile" unit. Seven men earned the Medal of Honor while serving with the 7th Cavalry in Vietnam.

On 2 May 1965 the battalion deployed to South Vietnam as part of the 1st Infantry Division. The battalion's DieHard Tunnel Rat section cleared the extensive Vietcong tunnel systems with little more than small arms and demolitions. 1966–1973: Approximately 5,000 US war dogs served in the Vietnam War (the US Army did not retain records prior to 1968); about 10,000 US servicemen served as dog-handlers during the war, and the K9 units are estimated to have saved over 10,000 human lives. 232 military working dogs and 295 US servicemen working as dog handlers were killed in action during the war. It is estimated that about 200 Vietnam War dogs survived the war to be assigned at other US bases outside the US. The remaining canines were euthanized or left behind.
